The short answer
Pick VoiceOS if you mostly dictate and want a voice agent that works through your apps, like Gmail, Slack, Notion and your calendar, for less money. Pick Koi if you want an assistant that answers out loud, points at your screen, draws the answer and clicks for you, with dictation that runs on your Mac and calls recorded without a bot.

Dictation
Both put your words where your cursor is, in any app. VoiceOS removes filler words and fixes grammar, offers polish styles, takes custom vocabulary and replacements, edits selected text by voice, and lists 100 languages. Its site says your audio is never stored on its servers unless you allow it to help improve the product, and that none of your data is used for training. It doesn't say where the speech is turned into text.
Koi's dictation runs on your Mac. Tap Fn + ⌥ and Whisper writes as you speak, offline, in 57 languages it tells apart by itself. The ums and stammers go, chats and code get no full stop at the end, names on your screen come out spelled right, and a word you fix once is spelled your way from then on. It's unlimited on every plan, and the audio never leaves the Mac. If dictation is all you need, our best dictation apps for Mac covers cheaper and free options.
The assistant
VoiceOS's line is “your cursor is the context”: point at something and ask. Its Agent Mode connects to Gmail, Slack, Google Calendar, Notion, Finder, Messages, Linear, Chrome, Google Drive, Google Docs and Obsidian, takes your own MCP servers, searches the web with sources and sets reminders. Its pricing page lists action confirmation. It doesn't say it clicks around your Mac or draws on the screen.
Koi is built around the screen itself. Tap Fn + ⌘ once and talk; it answers out loud and you go back and forth. Ask “which one do I click?” and it circles the button. Ask how something works and it draws a diagram over the screen. Say “click Skip” and it uses real clicks and keys, then checks that it worked. It reads your Mac's own apps when you allow it, today's calendar, unread mail, open tabs, a note, a contact, but it never buys, pays, sends, posts, deletes, signs in or types a password, and that's enforced in the app. See what Koi does on a call.
